Health Declaration and suitability & Pilates exercise
Before attending a Pilates class or personal tuition course you will have completed a Health Questionnaire & Enrolment Form or assessment. By gathering this information, the suitability, safety, and responsibility will rest solely with the client. Please make sure you have detailed any information on this form that will affect your ability to exercise safely.
If any questions on the Health Questionnaire & Enrolment Form have been answered as Yes, the responsibility lies with the client to gain approval from a health professional before attending. The participant will then need to update the Pilates Teacher of any health conditions before attending any form of exercises.
If your health changes at any time it is the client’s responsibility to inform the Pilates Teacher before attending a class or a personal appointment.
